Athletics-Physicals 2011

ATHLETIC PHYSICALS
If your child will be playing a sport for the 11-12
school year they will need a physical. Ohio
Athletics require each athlete to have a current
physical on file in order to try out or participate in
a sport as a 7th and 8th grader. Students must also
meet grade eligiblility requirements in order to
participate in sports. DON’T FORGET YOUR PHYSICAL FORMS ARE ONLY GOOD FOR 1 YEAR FROM THE DATE THE EXAM!
