Kindergarten Registration & Screening Print E-mail

Oregon City Schools has a full-day, every day Kindergarten program.  If your child has turned five by September 30 of the coming school year, you will want to attend the Kindergarten Parent Information Night.  Kindergarten Parent Information Night is held each year in March inform parents about Oregon City Schools and what is required to register an incoming Kindergarten student. 

 At this meeting parents must fill out a Kindergarten Registration Form  and schedule an appointment to complete the registration process.  Parents will receive a packet of forms that must be completed and submitted at the above-mentioned appointment.  Additional documentation must be submitted at the appointment (link to information) to complete registration.  Oregon City Schools also offers a Kindergarten Screen, which will take place in May.  Appointments are required for screening and are scheduled at the Kindergarten Parent Information Night. Oregon City Schools is excited to have your child join us!  If you have any questions, or have missed Kindergarten Parent Information Night and/or screening, please call Oregon City Schools’ Curriculum Office at 419-693-0661.

DOCUMENTATION REQUIRED FOR REGISTRATION

• Birth Certificate (See information below if you need a duplicate copy)
• Immunization Records (See below for information)
• Two (2) proofs of residency (ex. Utility bills, mortgage payment book, lease agreement or rental receipts)
• Custody Papers (If applicable – a complete “Filed” stamped copy is necessary)
• Completed Form Packet

 

Physician and dentist examination forms are to be completed and signed by the respective professional and brought to your registration appointment or returned to the school office as soon as possible. All forms are due prior to the first day of school attendance. Examinations must have been performed within one year of school attendance.

Information regarding Immunization Records:
Pupils enrolled in kindergarten through grade 12 are required to have written proof on file at their public or nonpublic school that they have been immunized against diphtheria, tetanus, pertussis, poliomyelitis, measles, mumps, rubella, Hepatitis B, and varicella (chickenpox) as set forth in Section 3313.671 of the Ohio Revised Code. Pupils who are not in compliance are to be excluded from school attendance no later than the fifteenth (15) day after admission. The minimum immunization requirements are:

Rubella
K-12
2 doses of MMR.  Dose 1 must be administered on or after the first birthday.  The second dose must be administered at least 28 days after dose 1.

Hib

None
HEP B
Hepatitis B
K-11
3 doses of Hepatitis B.  The second dose must be administered at least 28 days after the first dose.  The third dose must be given at least 16 weeks after the first dose and at least 8 weeks after the second dose.  The last dose in the series (third or fourth dose), must not be administered before age 24 weeks.

Varicella

Kindergarten
2 doses of varicella vaccine must be administered prior to entry.
